A Closer Look at Advance Care Planning: Your Role



This workshop aims to overcome barriers to practice by providing health professionals with the skills to instigate Advance Care Planning (ACP) discussions, equipping them with the confidence to facilitate family meetings and providing them with information to discuss the ethical and legal issues that are involved with end-of-life decision making. The workshop will provide health professionals with WA specific Advance Care Planning information, tools and resources.

Relevance to general practice
In a time of change around greater, client-centred care and decision making, Advance Care Planning (ACP) has become an integral part of general practice and primary health care in a move to improve patient outcomes (Mitchell GK, 2014). In Australia, the figures of Advance Care Planning completion are low despite support and legislations being put in place in every state and territory (Mann et. al, 2017).

Learning outcomes:

  • Identify the components of Advance Care Planning specific to individual need
    Demonstrate an increase in confidence to initiate a family conference for a palliative care case
    Discuss futile interventions, ‘at risk’ patients and the need for Advance Care Planning
    Demonstrate a greater understanding of the health professional’s role in end of life decision making
    Discuss the ethical and legal issues involved in Advance Care Planning

This education forms an Accredited Activity under the RACGP QI&CPD Program. Activity number: 125892
